Ask one question the product team can act on.

A cancellation reason is product research, not a gate the customer must pass.

Treat failed payment as a recoverable account state.

Involuntary churn needs a different flow from a subscriber who chose to leave.

A billing-recovery screen should state that payment failed, which plan is affected, whether access continues during a grace period, the next retry date, and how to update the payment method through the correct platform. Confirmation should restore a clear active state. Do not use cancellation language for a technical billing problem.

Send reminders in proportion to the consequence and user preference. Keep links secure and direct.
